Many patients leaving the hospital have limited access to the Internet. It is very important to provide patients with reliable healthcare information on which they can rely. In this assignment, you will assume the role of a discharge nurse who is tasked with providing a reliable diagnosis-specific website to support patient discharge education regarding their condition.
Choose only one (1) out of the four (4) most frequent hospital admission diagnoses below to be the patient’s discharge diagnosis:
Newborn infant
Pneumonia
Septicemia
Congestive heart failure
Section I: Written Paper
Write a one to two (1-2) page paper in which you:
Of the chosen patient diagnosis above, search the Internet, and identify the most appropriate diagnosis-specific trusted Website that you can give to the patient to support discharge teaching.
Describe the Website you have identified for the patient to take home, and outline the reasons, factors, and methodology you used to select this as an effective Website for your specific patient’s diagnosis.
Section II: Discharge Handout
Create a two (2) page patient-specific document which contains:
A basic explanation of the definition of the chosen diagnosis
A basic explanation of signs and symptoms in which the patient should call the doctor or go to the emergency room
A pasted graphical map to the nearest hospital (use any hospital in your local area)
A link and description of three (3) Websites for further information about the chosen diagnosis
Use at least three (3) quality academic resources in this assignment. Note: Wikipedia and other Websites do not qualify as academic resources.
Format your assignment according to the following formatting requirements:
Typed, double spaced, using Times New Roman font (size 12), with one-inch margins on all sides.
Include a cover page containing the title of the assignment, the student’s name, the professor’s name, the course title, and the date. The cover page is not included in the required page length.
