      Select a particular disease process (heart failure, diabetes mellitus, Parkinsons disease, etc.). Identify and fully describe a technology element that could be utilized in practice for a patient with the selected disease process. Describe how this technology will be integrated into the nursing workflow for treatment, monitoring, or communication from the acute care setting (hospital) and follow the patient to the home and ongoing care in the outpatient clinic.Please see attached directions
