Congestive Heart Failure

Congestive Heart Failure or choose a chief complaint that will help complete a SOAP note with a minimum of 3 differential diagnoses. (This is a created patient of your choice) 1. Develop a chief complaint - you will utilize this throughout the course - so make it a good one! Do not use a chief complaint that another student has already used/posted. 2. Document patient HPI (History of Present Illness) utilizing OPQRST (onset, provocative or palliative, quality or quantity, region or radiation, severity, and timing) 3. Document patient (from chief complaint in week 2) medical history, including previous diagnosis, year it was diagnosed, how it is being treated. 4. Document patient medication history – including WHY they take each medication. 5. Document patient allergy – including severity. We will build this patients SOAP note every week. So, make it count.
