are provided is there a kitchen and kitchen stafff and dietician or is food service contracted to outside
vendor? How are housekeeping services provided – in house staff or outside agency?
Financing and Reimbursement. Indicate if agency is for-profit or not-for-profit. How is agency reimbursed for
service delivery? What type of insurance is accepted to cover health services? This content should be
researched to add explanation beyond the interview.
Licensure, Certification or Accreditation. What is legally required for this agency to operate?. Include details
such as name of licensing agency and frequency of renewal. Do not include licensure for health care
professionals, as this is a known requirement for practice. Again, additional research my you will be
necessary.
Challenges. These include those challenges in health care delivery identified by the agency administrator,
e.g., staffing, technology, financing. Should NOT be the personal challenges to the administrator–such as, it
is difficult to fire an employee.
Personal Reflection. Should include: 1) what you learned by this assignment and how it relates to specific
topics of learning in the course (provide specific examples. Do not just say it related to all topics), 2) in what
way was the assignment meaningful to you, and 3) explain why or why not. Do not say, “I loved it” and leave
it at that. Give real thought to the reflection. This is the conclusion to the paper.
References. Note citations throughout the paper including: personal communication (who you interviewed),
agency web page (if available) and the evidence-based literature used to provide or support content in the

standard, traditional font (Arial) at a 12-point font size. Include page nuimbers.
Cover page MUST include the following: title of paper, your name, date the paper is submitted, course
number and title (CHP 390 U.S. Healthcare Delivery System), instructor’s name (Dr. Shuman) and ODU
Honor Pledge.
APA Style must be used for headings, citations and references.
