Within the report, you need to provide an analysis of the situation that discusses what happened and why it happened as well as your suggestion for how to keep the issue from happening again. If your example is from a text or the Internet make sure to include a link to the source or include it in an appropriately formatted References page. There is no length requirement for this assignment, but you must make sure to be thorough and clear in your analysis to ensure the professor has a full picture of the issue. Make sure to use a professional font, font size, and font colors. Since this is an informal report it does not require a title page or other elements of a formal report but it should still utilize headings, bullets, etc. as needed for readability.
