Annotated Bibliography Paper details: You may write about anything pertaining to nursing. You will have to use the PICO(T) for this. I will upload 2 documents. One is a "Evidence Based Practice Assessment-Searchable Clinical Questions Form" You will have to type into the tables that are listed. Also you you will see another form which is just a Guideline/Rubric it is titled "EBP Searchable Clinical with Annotated Bibliography Guidelines and Scoring Rubric". So the end result for this assignment will be the completed "Evidence Based Practice Assessment-Searchable Clinical Questions Form" and the Annotated Bibliography it self. The Annotated Bib should be 5 pages long, and form should with every thing filled may be about 2 pages. So i'm paying for a total of 7 pages.
