Answer How would you apply the Theory of Self-Care to a specific patient scenario that you have encountered where you currently practice as a nurse. Be sure to use Orem definitions and language in describing the patient situation.
The patient is a 22 year old female with past medical history of Rokitansky malformation sequence (status post renal transplant of immunosuppressive therapy). She presents with 1 week history of fatigue, shortness of breath, dry cough, diarrhea, nausea and vomiting and generalized body aches. She reports her symptoms are worsening and minimally improved with over the counter meds. She reports inability to take deep breaths secondary to pleuritic chest pain. Family members at home have had similar symptoms.
Past medical history: chronic kidney disease status post renal transplant in 2016, asthma, hypertension, acquired hypercoagulation deficiency, Rokitansky with multiple urogenital abnormalities including 4 ovaries status post bilateral oophorectomy for 2
Past surgical history: vesicostomy in 1998, anoplasty, vaginoplasty, 2 colostomies with takedowns, PDA ligation at birth
Allergies: augmentin, betadine, clindamycin, hydralazine, latex, oxybutynin, pediazole, sulfa, vancomycin
Right now: she’s intubated…having CMV viral pneumonia. Shes now being treat with linezolid and ganciclovir for her infection. Her lungs consistently has wet crackles sound. Shes stop making urine and now fluid overload also
