Health care quality across the United States is among the most important factors determining successful outcomes. The Centers for Medicare & Medicaid Services (CMS) evaluates a variety of metrics to ensure that hospitals are meeting their obligations to provide high-quality care. For this paper, we will be examining data from CMS’s National Quality Forum (NQF) 2019 Health Care Quality Report Card database.
In terms of overall health care quality in our state and nationally, we find that there were 24 measures tracked by NQF with seven core measures which served as “the basic building blocks for patient safety and quality measurement.” Of these seven core measure areas, our state ranked in the top 25 percent on four out of seven: mortality rate from sepsis, readmission rates after hip replacement surgery, preventable complications resulting from hospitalization (such as infections), and hospital-acquired condition rate. In addition to these findings, we also found positive trends in other measures such as preventing adverse events due to medication errors or pressure ulcers acquired during a hospital stay.
Based on this analysis, it is recommended that Oakridge Health System direct its focus toward improving its performance in the measure area regarding mortality rates from sepsis cases amongst Medicare patients admitted into an acute care facility within 30 days post discharge from another facility. At present, our system has successfully achieved a mortality rate of 14.5%, which surpasses both state and national averages; however, further improvements can help reduce risk associated with sepsis cases even more effectively than current efforts have already done so far . To accomplish this goal efficiently and effectively various quality improvement principles should be employed including: engaging all stakeholders; assessing processes currently being used; identifying gaps where improvements can be made through root cause analysis; designing novel interventions that are evidence-based; implementing them according to best practice standards while monitoring results regularly using collected data sets available through CMS's public facing online platform The Hospital Compare website ;and finally assessing any changes made over time by comparing pre/post intervention outcomes between control groups while taking into consideration potential confounding variables such as patient acuity levels or nursing staff ratios per shift at each site location etc..
By employing this systematic approach towards addressing morbidity/mortality related to sepsis cases amongst Medicare beneficiaries under Oakridge Health Systems' purview – they can not only meet but exceed established benchmarks set forth by CMS but also consequently improve upon overall health indicators seen within our given service area thereby significantly impacting long term population health outcomes positively throughout regionally .
