Purpose: The purpose of this project was to ascertain if quality of life is improved in ESRD patients after receiving a palliative care consult. The goal is to relieve the physical, emotional, and psychological symptoms suffered by ESRD patients, thus reducing hospital readmissions. Min objectives: ( actually I am not sure that objectives will • Develop a consultation palliative care /services program for ESRD Patient • provide advanced nursing care plan to prevent comorbidity • Explore the relation between the consultation care and quality of live • Effect of consultation care on delayed progression of ESRD and reduce the need for hospital admission • In addition, the government of Saudi Arabia has a vision (2030 vision) for the future of the Ministry of Health that aim to improve quality of life and health care services provided to the patients outside the hospitals. ( this is could be a rational for chose the topic ) The consultation program will be given to the patient regarding : Symptoms associated with ESRD ( pain, Dyspnia and monitor symptoms of uremic) , and management comfort and pain Control Blood pressure, Fluid and electrolytes as well as diet Skin care and medication compliance and psychological support, which are more focude on advanced nursing care plan. (for this part need focus on reach problem / problem statement , I put some instruction it could help you) 3- Proposed study design, with justification for choice. 4- Overview of proposed data collection methods and analysis techniques, with justification for choice. ( 3 and 500 words) It is important to present the proposed research methodology (e.g. techniques, sample size, target populations, species choice, equipment and data analysis) and explain why it is the most appropriate methodology to effectively answer the research question. Methods: For this project, every patient admitted to the Nephrology department (in patient) and Dialysis unit (outpatient) who is diagnosis as ESRD for 3 years or more at hospital received a palliative care consult. Participant criteria will include all patients have Hypertension or protein in urea), patient his prognosis more rapid (life expectance) or patient who has referral FOR END OF LIFE CARE (PALLIATIVE CARE). Quality of life will be assessed by administration of the TOOLS prior to the palliative care consult. The patient was then seen by the palliative care team while still hospitalized on the Nephrology department (in patient) or Dialysis unit (outpatient) . Quality of life will be reassessed using the SAME TOOL after discharge from the hospital OR AFTER COMPLETING NUMBER OF DIYALISIS SESSIONS. The QOL data will collected before consultation and after 2 month of consultation. ( need rational for chaos time and rational or justification for chose the methods) Tool used in study ( I am not sure which of the following Tools will fit my project) • Instrument World health organization quality of life (WHOQOL-BREF) questionnaire • HRQOL measure • Karnofsky Performance Status Scale (KPSS) • The Sickness Impact Profile (SIP) • The Kidney Disease Quality of Life (KDQOL) instrument (dialysis mea specifically developed for use with patients with ESRD treated with HD. Or could be select patient randomly and divided into two group ( control group and test group) test group will have the consultation for palliative care service /program then compared with other group).
