Journal Entry 7 Order Description From a nurse practitioner student in a family practice setting that uses Practice Fusion for electronic medical records (charting) : Reflect on how you assess patients and chart information at your practicum setting. Explain how you record patient history and physical assessment findings. Include whether behavioral and/or psychosocial assessments are performed and charted and how. Then, explain how your practicum setting is using patient information to improve care.
