Journal Entry 8 Order Description From the perspective of a nurse practitioner student in a family practice setting: Reflect on patients you evaluated at your practicum setting that you referred for specialized care or assistance (e.g., referrals to specialists, dieticians, social workers, etc.). Explain why the patient needed to be referred for care or assistance and whether the patient followed your recommendation. If not, consider what you might do next time to ensure patient follow-through. Then, explain at least two strategies to foster communication between patients and professional partners that provide specialized care and assistance.
