As a health manager you have been asked to design and develop a new model of patient education at the time of discharge with the aim of reducing the hospital readmission rate. 1. Identify which staff need to be involved in this new model and why? How will you involve them in this project? 2. What barriers are there to staff involvement? How will these barriers be overcome? 3. What planning processes will you use and why? 4. What types of information will you need and where will you source these from? AND 5. Provide an original diagram of the planning process for the new model.
