Focus group meeting at IDT meeting with NPs, pharmacy, market director to discuss project modalities including
– Discuss current DMP referral protocol. – Obtain consent to participate in the quality improvement project
– set project time frame
– NPs to choose from 18-75 years seen in the last 3 months with glucose greater than 200 or A1c >9 not referred to DMP using the protocol, recently discharged from
