Role of human factors in any chosen incident

In this task you are expected to visit any industry of your choice and conduct an incident investigation. Based on your observation, you are to prepare a detailed Health and safety Report. The report should consist of approximately 1500 words.
The Report Should Focus on identifying the role of human factors in the occurrence and consequence of the chosen accident. It should contain a brief introduction describing the background information of the case, identifying the specific pre-accident strategy and explain the features involved in each strategy with relevance to the case study, recommend post-accident (reactive) strategies fitting intended for the case, conclusion and supported references.
