the following scenario: You are a family nurse
practitioner who works in a low-income rural
health clinic. Many of the patients you see
struggle to make ends meet and often work
several jobs. They also typically have highdeductible insurance plans that make it
difficult to manage chronic care conditions.
Particularly, you see a high rate of type 2
diabetes mellitus among your patients.
Because they have limited resources and
invariable work schedules, many of them are
unable to purchase the medications they need
or comply with diet plans to help manage their
conditions. They also have difficulty keeping
follow-up appointments.
