A typed (5-6 page) report and a 5-10-minute class presentation are to be prepared. Information: Introduction, including a short summary of the type of case study. Include a listing of pertinent information regarding the department, size of the department and demographics, type (career vs. volunteer), victim information (name, age, rank, experience, training), and any other relevant background information, Incident summary, including a brief listing of the events leading to the fatality. Cause of death, if listed, the official cause of death should be given. In some case the cause of death may not be obvious. Detail the investigation results, recommendations, and discussion. The NIOSH recommendations should be listed along with a brief discussion of how and why these recommendations should be adhered to. Indicate whether these recommendations are part of NFPA 1500 or any other applicable safety regulation. Indicate whether your department, or the local department you choose, has these safety regulations in place and how they are addressed. Include any personal recommendations or observations that could be useful in the fire ground setting you may be involved in. The investigation report may not be duplicated and must be approved by the instructor. Students will be required to select their case study topic to be reviewed by the 2nd class date (A link to the case studies will be provided in Blackboard.) The paper must also have the following:
