graded. - Please avoid inserting opinion about your law. Your task is to address the topic factually, with no hint of your
possible support for the law. In the last paper, you will identify benefits and disadvantages of the paper…but
even that must be done with supporting references, not based on personal opinion. In an excellent paper,
actually should not be able to tell if you support the law.
Assignment: Addressing Patient Safety and Quality of Care Through Policy
Patient safety and quality of patient care are the focus of some of the largest changes within health care. IHI’s
Triple Aim initiative, as well as the Joint Commission and QSEN, are aimed at improving both the safety and
quality of health care. This Assignment gives you the opportunity to focus on your chosen policy issue for the
Position Paper and to explore factors that adversely impact quality and safety.
For this Assignment, review the following:
• Resources on quality/safety
• Resources on health care outcomes
To prepare for this Assignment:
• Review the health care policy that you selected in Week 1.
• Analyze your selected policy in relation to patient quality of care and safety.
• Relate your policy to IHI Triple Aim initiatives (found in the Resources).
• Examine the Assignment rubric for additional information and guidance.
Write a 3-page paper utilizing the health care policy you selected in Week 1 and 2.
Respond to the following prompts:
• In what ways does your policy address safety, quality, or improved outcomes of care for patients?
• Does your policy do an effective job of addressing the issue? Why or why not? Be specific and use examples.
• Explain the role of the nurse in addressing the quality and safety standards within your policy.
• Based on your assessment of the effectiveness of your policy on patient safety and quality of care, as well as
your research from Weeks 1 and 2, what position do you plan to take in your Position Paper (which is due in
Week 5)? Be specific on your viewpoint and reasoning.
• Support your position with scholarly references in addition to weekly resources.
Support your ideas or those of others with references from the professional nursing literature. Refer to the
Week 3 Assignment Rubric prior to submission.

• About Use of Direct Quotations and Past Work

Hello, class,
As you begin to develop your week 2 assignment, here are some very specific tips about the use of direct
quotations in this course. - Instructions for the discussions indicate direct quotations should be used in a limited way, and only if the
quoted content is “unique.” Instead, please paraphrase information from your readings to create your post. This
direction to paraphrase is intended to help you synthesize information and relay it in your own words. In a short
post, there also is no need for students to use direct quotations. Because the instructions are clear on this, I do
take off points from discussions when direct quotations are overused. - In a formal assignment, direct quotations can be used ONLY WHEN THEY ADD TO THE PAPER. For me,
this means when the direct quotation says something so uniquely that you could not possibly say it any better
or more powerfully. Please do NOT use direct quotations for statements of basic fact (e.g., “Osteoporotic
fractures affect about 300,000 people annually.”). A good paraphrase for this example might be: Fractures are

a major complication of osteoporosis, affecting about 300,000 persons every year. I do agree that occasional
use of such a direct quotation could be powerful. In my experience as a teacher and editor, however, a good
writer is able to paraphrase effectively to ensure a paper has his or her voice rather than use the words of other
authors. - Please be sure you do not overuse direct quotations. I cannot evaluate the quality of a student’s writing if a
paper appears to be a series of direct quotations largely strung together to create the required page length. I’d
say two or three at most could be appropriate in a formal assignment of two or three pages, but quotations
really need to add to the content or I will deduct points for overuse. Please also be sure to use a page number
(or paragraph number for an Internet source) for all direct quotations. As always, check APA information in the
Writing Center or use the APA Manual to ensure you are formatting your quotation correctly. - According to Walden University policy, students who wish to cite their past work (even in the same course)
MUST FIRST obtain permission from the current course faculty. If permission is given, students must also
appropriately cite themselves in the new assignment. Thus any student who uses previous material verbatim
without faculty permission is at risk for all that is implied by policy violation. Clearly, university leaders do not
believe that wholesale copying of previous assignments contributes to students’ increasing knowledge. I am
very unlikely to approve verbatim use of past work because I also believe students need to be synthesizing the
information as they move through a course, basically building on their knowledge as they continue to research
their chosen policy. I will not agree to the use of past work for a new assignment unless a student is able to
offer a convincing rationale for this.
I clearly can’t address what may have been allowed in previous courses. My consistent approach is to rely on
university policy and the resources provided for the course (e.g., Walden template, APA information in the
Writing Center).

I spend a great deal of time providing detailed feedback to help students improve their performance throughout
the course. In past classes, points have frequently been lost for formatting. PLEASE review the APA Manual or
APA resources in the Writing Center, as well as the tipsheet I emailed you comparing APA 6th ed. and APA 7th
ed.
You also should be able to access the completed rubric for your discussions and assignments, and a marked
paper for all assignments I have graded. If you have trouble locating those items in Blackboard, please let me
know right away so I can ensure you are able to read the feedback and continue to improve your scores.
Students in a 4000 level course should be able to - Write effectively. This includes proofreading assignments to be sure there are NO incomplete sentences, or
spelling or typographical errors. This also includes using SafeAssign to determine if a high percentage on the
originality report indicates too much material has been taken from other sources. You are welcome to revise an
assignment after receiving the initial SafeAssign report, and send it through SafeAssign again before
submitting. The focus should be on PARAPHRASING information from the course resources and other
references you use. - Use the APA Manual for all citations. APA formatting is like a formula, and it can be applied to any source
with only a little attention to detail. This includes correct citation of direct quotations in formal assignments; a
page number (print source) or paragraph number (Internet source) must be included with each direct quotation. - Follow instructions. For example, a 2-3 page paper should be a minimum of 2 full pages of substantive

discussion. A discussion is typically a minimum of 350 words. Failure to meet the minimum length for a paper
or discussion often points are lost because an in-depth discussion of the topic was not possible. I encourage

• URGENT Use APA Manual (7th ed.) for this course

Hello, class,
Just a reminder…you are now required to use the APA Manual (7th ed.) for your citations. Please be sure to
incorporate the new elements of the 7th edition in your work.
Some of the most notable changes include the following:
Three or more authors will ALWAYS be cited in the text with “et al.” This is a definite change from having to cite
up to five authors, but please incorporate this new requirement into your work. In the reference, up to 22
authors can be cited.
Publisher’s city and state will NOT be included in the citation of a book. Please only include the publisher’s
name.
“Author” will no longer be used when the author and publisher are the same.
Do NOT include “retrieved from” in the citation of an online source. Only the url should be provided.
