Behavior Change Project

Purpose: The purpose of this project is to explore the process of attempting to make a
positive change one’s health behavior. This is a semester-long project, which should be
started within the next week and conducted for the next 3 months.
Process:

  1. Determine a Target Behavior
    Choose a behavior that you would like to change. This can be:
    a) Something to eliminate (ex: cigarettes)
    b) Something to add (ex: broccoli)
    c) Something to modify (ex: increase your exercise routine)
  2. Pre-contemplation
    What factors have prevented you from making this change in the past?
    How long has it been since you seriously considered making this change?
  3. Contemplation
    In what ways will this change help you?
    Who will support this change?
  4. Preparation
    Detail the steps you will take to prepare for this change
    What rewards will you plan on for steps achieved?
    How will you handle a relapse situation (if you go off of your program)?
  5. Action
    Date you started the change
    Explain how you started your change
    How is your program progressing? (has it been successful?)
    What challenges have you encountered?
    Are you confident about successfully making this change?
