It is based on my experience in a school, and the child has to have any type of emotional or behavioral problems. Of what I observed & anything to do with a Behavioral Intervention Plan(BIP) and Functional Behavior assessment if it can be included. Only use first name of a child(any name/child). Child must be described in a inclusive setting classroom. Included should be a teacher and support team of the student. State opinion from parents of the child on how the child may act like at home. Then, compare and contrast home from school behavior or actions. The dates picked just leave blank I will fill it in. Need this before 11:00 so I can email it before midnight.
