List any indicators or information in th​‌‍‍‍‌‍‍‌‍‌‌‍‍‍‌‍‌‌‌‍​e assessment (or windshield survey) that you were surprised by and explain why. 5. What resources did you identify within your community that you can personally refer patient to? 6. What is your county’s plan to address the needs identified in this assessment? What are identified, and what is the framework used to address those needs.
