Identify one vulnerable population within a selected community. Explore current literature and related data to better understand the variables that place this
population at risk for health concerns and health disparities. Analyze national health directives (such as Healthy People or another national initiative) to
determine correlation with needs/risks/disparities of the vulnerable population.
Part 1: Concept Map
Identify the following elements to include in the concept map.
Vulnerable population
Variables (risk factors) that place the identified population at risk (minimum of three)
Minimum of one health risk or disparity for which the population is at risk
2. Create a concept mapthat portrays each element and the relationships between the vulnerable population, variables placing the population at risk, and the
identified health risk or disparity for which the population is susceptible. The concept map must be your own, original work.
