practice in the podiatry clinical environment.

Health education and promotion is seen as an essential component to effective management of the increasing
burden of chronic disease. The task for is to prepare a written proposal for the development of a
multidisciplinary self-care focused health promotion project within a Community Health Centre setting.
You are currently the only Podiatry staff member in the Community Health Centre in a small rural town. There
is also a GP, a Social Worker, Dietician, Physiotherapist and Community Nurse. Your eligibility criterion for
patient entry to the service is presence of one or more chronic diseases. Your criterion for continued treatment
within the service is set at high risk with active ulceration. You currently have two days of screening of new
patients with chronic disease who you then refer to one of the two local Podiatrists in town, but waiting lists for
those services are at least 3 months.
You are writing to the Allied Health Manager to justify your request for time to co-ordinate the program and for
any ongoing funding for incidentals such as catering, small pieces of equipment etc.
Your proposal should include the following:
• Evidence supporting the use of self-management education in chronic disease care.
• Evidence of financial gain to the health care system through implementation of self-care.
