1. Identify what you thought you would experience during the last two (2) weeks of the rotation, including your goals and objectives, and compare to your
actual experience. (My example for step 1: My expectations and goals were to learn more about IV compounding, become more familiar with the tasks in the
pharmacy, and become more familiar with drug names and their functions)
2. Summarize your activities and experiences during the last two (2) weeks of the rotation. (My example for step 2: I did record retention, assisted with IV
compounding, assisted with the maintenance of Ultra low freezers, assisted the pharmacist with priming the tubing set used for chemotherapy, observed the
pharmacist verifying prescriiption orders.)
3. Describe at least one (1) additional (and different) behavior or characteristic you observed among the pharmacy employees and how they positively or
negatively affect the delivery of services. (My example for step 3: The pharmacy staff worked well together to complete all the tasks.)
4. Describe at least one (1) additional interaction between pharmacy employees and each of the following healthcare professionals: Physician; Nurse.
Include your thoughts on what you expected to see as compared to how the interaction actually
occurred, a descriiption of the outcome and your thoughts on how the interaction could have been improved. (My example for step 4: The pharmacy staff
interacted with doctors and nurses in order to determine the correct dose of medication that the patient needs.)
5. Identify at least one (1) additional area of pharmacy practice you participated in that was covered in pharmacy coursework during your P1 year. (My
example for step 5: I learned about dimensional analysis calculations in my pharmacy curriculum and I used that knowledge to help prepare IV medications.)
6. Describe how you have been changed by this rotation experience and how it has helped you in your development as a pharmacist. Include an assessment
of strengths and weaknesses you identified in yourself during this rotation. (My example for step 6: I learned a lot of pharmacy skills during these rotations
and it has positively impacted me in my journey to becoming a pharmacist. My strength is the ability to work with other employees. My weakness is not
having complete pharmacy knowledge.)
