Malnutrition in older adults

    Older adults are at risk for malnutrition due to age-related changes, medical conditions, and medications. Identify a patient that you have seen in clinical who has or is at risk for malnutrition. Discuss your patient and their risk factors for malnutrition. Identify one assessment tool that you can use to identify malnutrition in this patient. Identify an evidence-based plan for this patient to either treat their malnutrition or intervene to prevent malnutrition.
