Mass screening program

Case Study 1
J.L. is a 56-year-old woman who comes to the Nurse Practitioner’s office because she is feeling tired. You are the registered nurse working in the office and the Nurse Practitioner has asked you to complete a health history and nursing assessment before she sees the client. You enter the room and begin your assessment of J.L. She tells you that she has lost about 35 lbs in the last two months and has been on a diet, tracking her calories and weighing her food. She is surprised but pleased that the weight came off so fast because she has not been exercising. J.L. states that she is very tired and commutes to and from work on the train. Her job is very stressful. She had surgery to remove a tumour from her colon in March and now, six months later she feels glad that she had the surgery. She has no family history of colon cancer that she knows of, but she is not certain because she was adopted as a baby and did not know her biological parents. J.L.’s recent blood test suggest an abnormal CBC, and low WBC count. A colonoscopy was completed post visit to the clinic. She has also had a CT scan. The results came back indicating that she has an aggressive form of colon cancer that has metastasized to other organs. The nurse practitioner has met with the client and informed her of the diagnosis and is referring J.L. to an oncologist at a local cancer centre.

How would you define a mass screening program? Are mass screening programs effective? If yes, effective for what? What are the potential clinical (population based) and financial implications of this? Would client in Case Study 1 have qualified for a screening program? Explain your ideas in your own words.
