Written Report Requirements________________________________________
I. The report must be written in APA format and will include a cover page, and reference page.
Topics to choose from:
a. Opioid/Opiate Dependence,
b. The Addicted Neonate (concentration on addiction at birth), or
c. Alcoholism.
II. Include all the aspects of the Medical Model format. It must be typewritten, double spaced. The body of the
report is to be no less than 4 pages and no greater than 6 pages in length. Use this link to access Library
Services which will assist you in using the internet references for scientific inquiry.
III. The Tutoring and Academic Support Center is available to assist with the writing of the report and
accurately citing references (within the body of the paper and your reference list). Be sure to use quotation
marks when quoting a reference – plagiarism is a violation of Bristol Community College’s policies. Refer to the
course syllabus for contact information for tutoring/academic support.
IV. References are valid, reliable and current (within the last 3 years).
V. The following areas within the Medical Model must be addressed:
a. Name the condition and define it (Diagnosis and definition of the diagnosis).
b. Etiology and Risk Factors: cause(s) of the condition and the factor(s) which place one at risk.
c. Epidemiology: the study of the populations of those with the condition. Please, report on the statistics in your
area (ie: Fall River, New Bedford, Taunton, etc.)
d. Describe the pathophysiology involved: the anatomical and/or physiological changes which occur within the
body due to the condition. These changes are responsible for the manner in which the condition manifests
itself.
e. Procedures to diagnose the condition: laboratory and/or procedural tests to determine and prove the
existence of the condition. How is the condition diagnosed?
f. Manifestations: the signs and symptoms of the disease. The way the disease makes itself known.
g. Treatment options: measures to cure, mitigate, or soothe the condition.
a. Drugs: specific medications (generic and Trade). Major side effects are to be included.
b. Nutrition. Specific foods/fluids, supplements used as a treatment.
c. Rehabilitation. Options to promotion a return to a healthful state and/or mitigate the condition.
d. On-going needs.
h. Potential complications: new conditions, which may appear as a result of the condition. (If choosing the
addicted neonate, note the problems that may occur as the infant grows to adulthood.)
i. Prognosis: a prediction of the outcome of the disease process. What is the long term expectation?
j. Prevention: an action to keep something from occurring/recurring in the future
k. References (APA format).
