Patient’s diagnosis

The client is a 56 years old Latino male presented with a history of Major Depressive disorder PTSD and substance use disorder. The client’s oldest son Jerry is 30 years old and also attended the therapy session. Client endorsed smoking marijuana daily. The son indicated that the client is unable to take care of himself. He has been having a lot of fights with two daughters Amy and Debie regarding the recent change in his behavior. The son stated I am depressed and overwhelmed by all these things.

CC: ” I want to be sober. Since I lost my wife 5 years ago in a MVA, nothing is the same anymore. I used marijuana to numb my depression. My kids won’t understand. I have nightmares every night, flashbacks, intrusive thoughts and sometimes I am disconnected from reality. I don’t have pleasure in doing anything. I just want to smoke and be in bed.
Then, address in your Practicum Journal the following:

Using the Group Therapy Progress Note in this week’s Learning Resources, document the family session.
Describe each client (without violating HIPAA regulations) and identify any pertinent history or medical information, including prescribed medications.
Using the DSM-5, explain and justify your diagnosis for each client.
Explain whether solution-focused or cognitive behavioral therapy would be more effective with this family. Include expected outcomes based on these therapeutic approaches.
Explain any legal and/or ethical implications related to counseling each client.
