Planning for program proposal

Thinking about p, please describe the first 4 phases of the Precede-Proceed
Model. As you describe each phase, describe the kinds of information you would need for assessing need for
the project you are developing/describe the kinds of questions you will ask for your project and how you might
acquire this data? (Hint: some of these elements are most readily explored with existing secondary/tertiary
data, while others may require key informant interviews or surveys/focus groups with members of the target
population.)
In the Community Toolbox, the Precede-Proceed model is described as participatory. Sometimes needs
assessment and program planning occurs at the community level–such as a Community Health Needs
Assessment and Community Health Improvement Plan—-and sometimes it is at the level of a smaller scale
grant funded intervention. Describe how you might use the precede-proceed frameworks or similar model to
guide a community participatory process for a smaller grant funded intervention. (Hint: community participation
could occur for the overall planning as key data for at least two of the first four phases.
