Patients typically report that they have been told by their medical provider that they should switch to a healthier diet, exercise more often, or start taking insulin therapy but feel overwhelmed with the diagnosis and unsure how best to follow this advice. The primary aim of this project would be to assess whether implementing an evidence-based patient education program can improve glycemic control among these newly diagnosed patients during their first year following diagnosis. The intervention will focus on providing resources such as online webinars on nutrition and physical activity tailored specifically for those managing type 2 diabetes in order to increase knowledge about lifestyle changes related to better management of their disease. Additionally, it will include regularly scheduled phone consultations with a healthcare provider as well as access to peer support groups for additional guidance and motivation in forming healthy habits associated with effective diabetes self-management strategies over time. A preliminary needs assessment allows for the identification of gaps in patient care and an opportunity to develop a plan that can address those gaps. In order to conduct a needs assessment, I will use the information from modules 1-3, which include evidence-based literature reviews and healthcare guidelines related to diabetes management and prevention. From this data, I will be able to identify any discrepancies between current practices or protocols and best-practice standards recommended by experts in the field. Furthermore, I will explore any potential risks associated with inadequate diabetes care as well as evaluate resources in terms of availability or accessibility for new patients who may require additional services such as dietary counseling or physical activity advice. Finally, I will assess whether there are specific cultural considerations that need to be taken into account when designing my intervention program.
