Summarize the patient, family, or population problem.- Hypertension. To include why Hypertension is such a major health deficit and why is Hypertension such a big problem in the US?
Explain why the problem of Hypertension is so highly relevant in the hospital environment and why interventions elicited by the nurse are so important.
Below is the multipronged outline for my targeted Hypertension treatment and education for my individual: Mr. BN. Hispanic male/68 y/o
1. Medications
a. Explain the best classes of medications for the control of hypertension and risks associated. What are the best choices for your specific patient/client?What are the practice standards in prescribing hypertensive medications? (history of patient: Hispanic Male, initials-BN, 68 y/o with a diagnosis of hypertension for multiple years, non compliant with medications as well as not following guidelines to decrease the modifiable risk factors associated with hypertension)
Examples: (Ace inhibitors, ARB’s, Calcium channel blockers, thiazide diuretics, beta blockers)
a. Explain how certain over the counter supplements such as Fish oils, coq10, probiotics, roselle(hibiscus), red yeast rice, beets and garlic have a positive effect on reducing high blood pressure. What are the risks of taking OTC supplements for High blood pressure?

2. Lifestyle changes
a. Explain how daily monitoring(1, 2 or 3x/day) via blood pressure monitor at home increases not only patient compliance but also actively involves the patient to become empowered to reduce their blood pressure.
b. Explain how a smartwatch increases patient compliance to an exercise regimen. And what are the recommended daily steps or exercise regimen for improving health and specifically lowering blood pressure. How does exercise assist in lowering blood pressure? How does a sedentary lifestyle affect blood pressure?
c. What dietary changes will assist in the reduction of high blood pressure: Incorporating the following: Foods high in C, E, K, selenium, L arginine
d. Smoking cessation-Why is smoking cessation so important in reducing high blood pressure? What are the best methods/medications to reduce smoking?

● Explain how your Lifestyle modifications such as dietary changes and exercise will improve the quality of care, enhance patient safety, and reduce costs to the system and individual.
○ Cite evidence from the literature that supports your conclusions.
○ Identify relevant and available sources of benchmark data on care quality, patient safety, and costs to the system and individual.
● Explain how technology, care coordination, and the utilization of community resources can be applied in addressing the problem.
○ Cite evidence from the literature that supports your conclusions.
