I am aiming for an excellent or an outstanding , the wording and the level will barely get me a thresh-hold , As I mentioned this has 90% of my grades so I need it to be done properly 3. the introduction must explain what model is being used for each incident , avoid using too much abbreviations , All SMART&. SWOT must be referenced 4. I need an overall conclusion the brings in all the Aspects of The document and goes through. 5. Each scenario should be descriptive no need to mention un-neccessary informations like what an OSCE is 6. Throughout the the reflection all references should link between theory and practice eg:( I was counselling the patient and explained verbally how to use the inhaler , reflecting on that, I shoud have used an actual inhaler to demonstrate how the patient should use. It as it states in James et. 2018) that using visual aids help patients understand better) 7. Smart plan should be objective , the swot should elaborate for the smart ( all based on the incident) 8. Each scenario should explain 1 root problem and how its linked to the attributes and EPAs, not a general scenario each scenario should explain how I demonstrate the pharmacist roles (collaborate negotiator resilience person centred care etc.) 9. You need to be using more good quality references and making links between theory and practice. Scenario 1 : counselling a dialysis patient, I introduced myself and we went through the medication prescribed , the patient was not adhering to one of the medications prescribed which was a cinacalet because of the side effects being nausea and vomiting and he was complaining of being in the hospital vomitting the past day and he did not want to take the medication. I explained to him the correct way of taking the medication and the importance of taking it following the prescribers directions, I also ensured him that his issue is taken seriously and that the prescriber will be contacted regarding his discomfort and advise will be given to to find a better solution. Must explain how I demonstrated the pharmacist roles and attributes( collaborator , negotiator , educator, person centre care ) etc., read the EPA wording and explain how I used the roles , must link of “proof” with no commentary or evaluation. Follow this with the influencing factors what internal/external/knowledge affected your decision making? Consider what other choices did you have and what were those consequences? How about your learning: What will change because of this experience and how did you feel about the experience How has this experience changed your ways of knowing Empirics –scientific Ethics –moral knowledge Personal –self awareness Aesthetics –the art of what we do, our own experiences Scenario 2 :failing my OSCE in second year because I dispensed the wrong type of medication , the prescription said metformin but I dispensed metformin Modified Release which leads to patient harm which lead me to now always double check the prescription ,label and. Medication and in practice if I’m in doubt always ask Recommend. (Premise) Objective should not include learning or data that was part of the initial experience but additional research, feedback or opinions/interpretations you sought when you realized you could learn more from the experience. Reconsider the experience from multiple perspectives. Go beyond imagining others’ perspectives to obtaining data: 1) Use open-ended, open-minded questions to elicit opinions, interpretations and feedback from mentors, supervisors, other professionals, patients, families, and/or peers, and/or 2) consult the medical literature or other sources of relevant information. Useful objective data will help you reframe your understanding the experience and deepen your learning. Assessment requires analysis of the experience to integrate the subjective and objective data and current with past experience. It should result in new understanding or identification of new learning goals with relevance for you beyond the specific event described in the subjective section. Synthesize your learning: What educational, personal or professional challenges and/or strengths have you identified? How did this experience relate to your past experiences? What personal/professional patterns have you identified? How has this analysis affected how you will approach similar situations in the future? Look for larger learning/ professional development issues. Specify lessons learned or questions/learning issues identified. Plan should consist of action items which can be accomplished and evaluated in the upcoming weeks to months and can be revisited with an advisor, mentor or peer group to track your professional development and to make sure you are reflecting in a way that informs your learning and/or clinical practice. Make a plan to address future similar challenges. The plan should be SMART: Specific, Measurable, Attainable, Relevant and Timely. The plan should include: a) what specific next steps you will take; b) where you can get the information or help you need; c) who you will check in with and when; and d) how you and your check in person will know whether or not your plan is working. If the plan is SMART, you should be able to assess the utility of your action items for furthering your learning or practice in days to weeks or months at the most. I hope this. Is.
