?Think back to an incident that affected patient safety, had unintended outcomes, or did not contribute to the well-being of the patient.
?Reflect on the incident and what and why the breakdowns occurred.
?Describe how you would as a manager/ supervisor create an environment where these incidences are less likely to happen.
Include a minimum of two scholarly sources.
Please writer add an introduction at the beginning of this essay and a conclusion at the end of the essay. Refer to Box 5-4, Guidelines for Analyzing Critical Thinking (Rubenfeld & Scheffer, 2010, p 119-120)
